I think it's because it's your weird holding of the paddle, I remember you saying it was some crazy way. For forehand blocknig it shouldnt be just your forehand sticking out, rather it's kind of like a forehand hit but just very small movement. Here's an example; look at schlager as wilson zhange loops.

Good tip
always start yr fh warmup just steady hitting. After you are warm and have feel of table THEN start topspinning.
Thats what Schlager recommends.
Start easy to get your eye in.
Angle of racket is up to you, though too much closed is the usual error from those that think topspinning EvERY
ball is good TT

the angle depends on the amount of topspin, if there is a crapload, lower it if there isnt any make it higher whenever people block while practicing it's never just standing there with your paddle moving. Even when penhold players play they push a little when they block.

Try playing with short pips for a while. You will definitely improve on your hitting. Then you can go back to playing with inverted. It helped me a lot.

Counterhitting is about 90 degrees, flat hit, just hitting it back.

agreed with the 90 degree comment. You need a cooperating practice partner who's counter hitting too.

I suggest playing around with the angles a bit.  And don't be timid, don't be afraid to swing at the ball a good deal.

swiffers thats   my problem, i am afarid to swing the ball, and my wrist moves, which then makes the ball go outside.
